Admission Process

Step 1

If you are interested in applying to Usha Martin School, Malda you can opt for either of the following options:-

1.Fill up and submit the Online Application Form on our website

Or

2.Download the Application Form, fill it up, and submit at the respective school address.

Step 2

Once the application is submitted , our admission office will arrange an interview/assessment for the child and parents .

Step 3

Payment of Admission and other fees including Security deposit will have to be made to the School City Office by cheque/pay order/draft along with submission of the following documents:
1. Original & photocopy of address proof of parents and/or guardian
2. Passport size photos (6)
3. Original and photocopy of applicant’s birth certificate duly attested by a Gazetted Officer
4. Original & photocopy of the Adoption Certificate in case the applicant is adopted
5. Original & photocopy of legal documents of custody of child in case the parents are legally separated
6. Original & Photocopy of Medical certificate regarding the physical fitness of the child and or history of vaccination
7. Transfer certificate of previous school, in original
8. Attested photocopy of last progress report (not applicable for Pre-primary)

Note: Originals, in case of no. 2, 4,5 and 6 above, will be returned after verification

Step 4

You can apply online for the Prospectus. You will receive a confirmation mail. We will be in touch with you within the next couple of days.
